​​Acute Illness Care:
Neighborhood Health has developed a full-range of primary care services for chronic disease provided within our own health center and through referral arrangements. Our providers use the full spectrum of primary care services necessary to manage the chronic illnesses that are prevalent among adults and children in our community. Neighborhood Health providers follow current guidelines from the American Diabetes Association (ADA), American Academy of Pediatrics, American Heart Association (AHA), American College of Cardiology (ACC), and the National Heart, Lung, and Blood Institute (NHLBI) in managing these conditions. Neighborhood Health’s chart review process ensures that our clinicians care is in accordance with these guidelines. In addition, case management by staff nurses ensures that patients have access to the full range of health and wellness services for which they are eligible.​​

General Medicine: 

Our provider’s are board-certified physicians, nurse practitioners, and nurses who can provide comprehensive primary care and medical specialty services to individual over 18 years of age. Patients are seen for purposes of receiving primary medical care, if necessary, referrals are made.

Routine Physical Exams: 

The Routine Physical usually consists of examining the patient’s history, looking at vital signs, patient general appearance, heart, lung, head and neck, abdominal, neurological, dermatological, extremities, and laboratory tests.

​Case Management:  
On-site case management services are integrated into Neighborhood Health's primary care health delivery system. Our nursing staff engages in case management in conjunction with medical providers. Patients referred for radiologic tests, laboratory tests, and specialty care that needs follow-up are tracked by nursing staff in collaboration with the provider. Abnormal cancer screening tests for cervical, breast and colon cancer will be tracked by designated nurse case managers who ensure that patient results are managed in a timely manner with either follow-up or referrals for specialty care.
